Project Focus
Provides ambulatory detoxification and intensive outpatient substance abuse treatment services to a select group of 12 men who reside at Jefferson Place 12 months. A collaborative effort between Fulton County’s Human Services Department and the Department of Mental Health, Mental Retardation and Substance Abuse.

Case Management
A case manager and the consumer jointly identify specific social services needs and assess how much or what sort of assistance is required to meet these needs.

Resettlement Assistance
Helps participants of the Jefferson Place Supportive Services Program obtain permanent housing and resettle into the community. Eligible consumers must be working full-time for at least four consecutive weeks and demonstrate the financial ability to continue to pay rent and utilities for the unit being considered for occupancy. Consumers must also be actively fulfilling the requirements and obligations of their individual service plan which is designed to assist in the transition from homelessness into permanent housing.

Needs Assessment
A needs assessment is completed with each consumer and an Individualized Service Plan (ISP) is developed. The major service needs addressed in the ISP are: housing, income, transportation, substance addiction services, mental health services, medical services, credit and money management and other social services (vocational, career training).

Follow-Along Services
Follow-Along contacts are maintained for all consumers who have successfully completed the program at Jefferson Place. The Case Manager seeks to have contact (face-to-face or telephone) with the consumer at least twice per month for the first three months and then once per quarter for three quarters. Follow-Along contacts are supportive and attempt to obtain information relative to job situation, living conditions, life changes, usefulness of Jefferson Place programming and the current need for additional services.

Fulton County Collaborative to Reduce Homelessness
This monthly forum provides information sharing and networking opportunities to Fulton County service providers with a focus on homeless prevention and intervention strategies. The Collaborative is open to nonprofit organizations, public agencies, advocates, concerned citizens and other interested community stakeholders that address the issue of homelessness in Fulton County. Special Events
The staff and consumers of Jefferson Place assist and participate in a variety of Special Events throughout the Fulton County community including: Red and White Ball at the Bowden Senior Multipurpose Facility, Halloween Community Festival, Art for AIDS, Annual Emergency Shelter Thanksgiving Dinner, Christmas Celebration, Sobriety Birthdays and Alumni Meetings.
